In January 2021, fourteen safety net clinics in the Rio Grande Valley began to screen their patients with a standardized instrument for social determinants of health. The instrument contains 19 items. Patients take the questionnaire as part of their routine appointment intake workflow. 

To date, there are 4,417 responses to the screening tool. The tool is also available in Spanish and has a dashboard identical to this one. 

In the dashboard, you will see a chart that shows where the responses to the screening tool were collected. Then, you will see the responses to the questions in the screening tool.
