Launched in the fall of 2002, the plan, Moving Our Children Toward a Healthy Weight: Finding the Will and the Way, calls for a multilevel approach to reducing the number of overweight and obese children. It focuses not only on behavioral and interpersonal change, but also on the organizational, community, and societal changes necessary to support healthy eating habits and increased physical activity for children, teens, and their families.

The Community Preventive Services Task Force (CPSTF) recommends self-measured blood pressure monitoring interventions combined with additional support to improve blood pressure outcomes in patients with high blood pressure. Additional support may include patient counseling, education, or web-based support. Economic evidence indicates that self-measured blood pressure monitoring interventions are cost-effective when they are used with additional support or within team-based care.

The goal of this program is to increase provider recommendation and patient compliance with colorectal cancer screening at a federally qualified health center serving low-income patients.
The intervention appears to be a feasible means to improve colorectal cancer screening rates among patients served by community health centers. However, more attention to patient decision making and education may be needed to further increase screening rates.

Immunization rates increased from 53.2% to 72.4% and parents reported a better understanding of the importance of timely vaccinations.

Pathways to Housing has received numerous awards for its projects in New York City, Philadelphia, and D.C. In addition to achieving large effects in terms of housing outcomes, compared with standard care in the community, Housing First also improves community functioning and quality of life.
